    • Test Execution Progress: If execution for high-priority test cases is incomplete, or if the pass rate is significantly below our quality gate threshold, it indicates instability.
    • Defect Analysis: Crucially, we identify open high-severity/priority defects impacting core functionality, particularly those found late in the cycle through exploratory testing. A rising Defect Reopen Rate suggests underlying quality issues or ineffective fixes, while a high projected Defect Leakage Rate from prior sprints indicates potential for customer impact.
    • UAT Feedback: A low UAT Pass Rate from business users is a strong indicator of unmet expectations or critical functional issues discovered during manual user acceptance testing.
